What are Nipple Tattoos?
Nipple and areola tattoos are medical tattoos designed to appear three-dimensional or as similar to a natural nipple as possible. An experienced tattoo artist can create truly realistic results that help patients feel whole again.
There are many advantages to choosing nipple and areola tattoos following a double mastectomy or breast reconstruction surgery. This technique avoids the need for additional plastic surgery and an extra healing journey to create a nipple, while the associated recovery process is quicker and easier than surgery. Skilled tattoo artists can also add fine details and coloring to the nipple and areola area, achieving a natural appearance that cannot be replicated through surgery alone.
Who is a Candidate for 3D Nipple Tattoos?
Individuals who have undergone breast reconstruction or breast cancer surgery with an implant or DIEP flap reconstruction may be candidates for nipple and areola tattooing. Patients who have had unsuccessful nipple reconstruction surgery can also benefit.
Candidates should be fully healed from breast surgery, with no open wounds, scabbing, drainage, active irritation, or signs of infection. Tattooing is generally considered one of the final stages of reconstruction and is commonly performed at least three to four months after surgery. However, the appropriate timing depends on the patient’s healing progress, treatment history, and recommendations from their surgical and oncology teams. Patients who have received radiation therapy may require additional healing time before tattooing.
However, some patients may not be ideal candidates, including those with a history of infections in the breast area, lymphedema involving the chest, very thin skin due to mastectomy, or breast skin affected by radiation treatment. A consultation with a skilled nipple tattoo artist is the first step toward determining candidacy.
Tattooing may need to be postponed for patients with an active infection, unhealed incision, significant skin breakdown, uncontrolled bleeding disorder, or a medical condition that interferes with healing. Certain medications, including blood thinners or treatments that suppress the immune system, may also affect candidacy. Patients should not discontinue prescribed medications without first speaking with their healthcare provider. Medical clearance may be requested when there are concerns about the skin’s condition or the patient’s ability to heal safely.

The Nipple Tattoo Process
Nipple and areola tattooing is often the final stage of breast reconstruction and is usually scheduled at least four months after surgery. This process typically requires two appointments, the initial tattoo session and a follow-up touch-up to perfect color and detail.
The process begins with a consultation to discuss your surgical and medical history, healing progress, aesthetic preferences, and any concerns about the treatment. Dawn will evaluate the skin and work with you to select an appropriate size, shape, color, and placement. For unilateral tattooing, the design can be customized to complement the natural nipple and areola on the opposite breast.
Appointments usually take two to three hours. During this time, you will discuss your preferences for nipple and areola size, shape, and placement. The tattoo ink will be customized for a natural appearance based on your skin tone. Since reconstructed breasts often have reduced sensation, tattooing is generally not painful.
Once the design is approved, layered pigments and specialized shading techniques are used to create the appearance of natural dimension. Although the tattoo looks three-dimensional, it remains flat against the skin. Some patients may experience mild sensitivity or discomfort depending on the amount of sensation present in the treated area.
After the procedure, your tattoos will be covered with a protective dressing, and your tattoo artist will provide instructions for care. The color of your tattoos may fade slightly over time, and a touch-up session may be recommended.
Patients should follow all aftercare instructions, which may include keeping the area clean and protected, avoiding soaking or swimming, and limiting friction, sweating, and direct sun exposure while the skin heals. Mild redness, tenderness, swelling, or temporary darkening of the pigment may occur. Patients should contact Dawn and their healthcare provider if they develop worsening redness, warmth, drainage, fever, or other possible signs of infection.
The surface of the skin generally heals over the following weeks. Tattoo pigment initially appears darker and becomes softer and more natural-looking as the area heals. A follow-up appointment is typically scheduled after the skin has had time to recover so Dawn can evaluate pigment retention and refine the color, shape, or detail. The final result is assessed after the tattoo has completely healed and the pigment has settled.
What to Expect

- Custom Color: Tattoo pigments are blended to match your skin tone for a natural look.
- Comfort: Most reconstructed breasts have reduced sensation, so the procedure is generally not painful.

After Your Tattoo:
- A protective dressing will be applied.
- You will receive detailed aftercare instructions.
- Some fading is normal between sessions, and the second appointment helps ensure lasting, natural results.
- Avoid picking, scratching, or removing healing skin, as this can affect pigment retention. Follow Dawn’s instructions regarding bathing, exercise, clothing, and when you may resume swimming or other activities. Once healed, protecting the area from direct sun exposure can help preserve the tattoo’s color.
Scar Camouflaging
In addition to nipple and areola tattooing, Dawn Morrow also offers scar camouflaging, a specialized tattooing technique that blends scar tissue with the surrounding skin tone to help minimize its visibility and create a more natural appearance.
Scar camouflaging can be performed on a wide range of areas, including but not limited to:

Scar camouflage is generally performed only after the scar has fully matured and stabilized. Dawn will evaluate the scar’s color, texture, location, and healing history during a consultation to determine whether tattooing is appropriate. Raised, irritated, unstable, or actively changing scars may not be suitable for treatment.
Accessing Insurance Benefits for Nipple Tattooing
The WHCRA requires that insurance providers cover all stages of breast reconstruction after a mastectomy, including nipple and areola reconstruction. This means that nipple tattooing is a covered benefit for many women undergoing breast surgery. However, it’s important to note that not all insurance plans are created equal, and coverage can vary depending on your specific plan.
The Women’s Health and Cancer Rights Act applies to many group health plans and health insurance issuers that provide mastectomy benefits. Coverage remains subject to the patient’s plan requirements, including deductibles, copayments, coinsurance, network restrictions, prior authorization rules, and medical-necessity criteria. Coverage may also depend on who performs the tattooing and whether the provider can bill the insurance plan.
Before scheduling, patients should call the member-services number on the back of their insurance card and ask whether medical nipple and areola tattooing following mastectomy and breast reconstruction is covered.
