Rather than treating tumor removal and cosmetic outcome as two separate steps, an oncoplastic approach considers both from the very beginning of surgical planning.
In practice, this means Dr. Ley plans the incision, tissue rearrangement, and closure in a way that removes the cancer with clear margins while also reshaping the remaining breast tissue to minimize distortion. Oncoplastic techniques are used both during lumpectomy (partial mastectomy) and, in some cases, to improve the final cosmetic outcome of a mastectomy.
Level 1 and Level 2 Oncoplastic Techniques
Oncoplastic approaches generally fall into two levels of complexity.
Level 1 Oncoplastic Lumpectomy This is the simplest form of oncoplastic surgery. It involves careful, aesthetic scar placement and rearrangement of the tissue near the tumor bed. After the cancer and a margin of healthy tissue are removed, nearby tissue under the breast skin is loosened and used to fill in the resulting gap. The scar can often be hidden around the areola or under the breast, with incision placement varying based on where the tumor is located.
Level 2 Oncoplastic Lumpectomy These are more involved techniques for reshaping breast tissue, and may include a breast lift or breast reduction performed alongside the cancer surgery, volume replacement using nearby tissue flaps, or volume replacement with fat grafting. Because of the added complexity, Level 2 procedures may require Dr. Ley and a plastic surgeon to work together to achieve the desired result.
Regardless of technique, the surgeon takes care to preserve the nipple and areola’s blood supply, adjusting their position as needed.

Recovery After Oncoplastic Surgery
Recovery from oncoplastic lumpectomy is generally similar to recovery from a standard lumpectomy, though the exact timeline varies by patient and by the extent of tissue rearrangement involved.
The First Few Days Most patients experience some swelling, bruising, and mild discomfort, which is typically managed with prescribed medication. Activity restrictions, particularly around lifting and upper body movement, are common in the initial days after surgery.
Weeks Following Surgery Swelling and bruising gradually subside over the following weeks. Final results, including symmetry and contour, continue to settle over the following months as tissue heals.
